Grant evaluations must be received by the end of the academic year in which the funding was used.
Evaluations should contain the following information:
1. Program Title and name of person who submitted the application, as well as contact telephone number and e-mail address. Include actual program dates or timeline.
2. Name of person completing the evaluation (if different from the person who submitted the proposal). INCLUDE ALL CONTACT INFORMATION, including phone number and e-mail address.
3. Analysis of the effectiveness of the project's activities and whether the goals and objectives of the project were met. PLEASE BE SPECIFIC, rather than general.
4. Description of any information that was gathered and recorded before, during and after the project.
5. Student assessments, where appropriate. These are particularly useful.
6. A summary of actual grant expenses.
